On-Site Attendance Form

Families who have a child that falls under one of the 3 categories for needing supervision on-site, may request for the school to provide supervision during remote and flexible learning.

It is the parents/carers responsibility to ensure that the supervision form is received by the school no later than the Friday before a remote and flexible learning week commences. If you do not submit a request for onsite supervision, you will need to organise alternative supervision arrangements for that week.

It is the Principal's responsibility to ensure that enough staff are on-site to supervise students who are at school. This can only be done when attendance request forms are handed in. It is not the school's responsibility to chase up missing attendance request forms.

How do I know if my child is eligible to be considered for attending onsite?

I am an authorised worker:

My child/ren is/are experiencing vulnerability:

  • Goverment agencies such as DFFH, Anglicare, OzChild, etc. can request directly to the school on your behalf.
  • If you believe that your child is vulnerable and are not linked in with a government agency, please phone the school to discuss this further.

My child has a disability, can they be supervised onsite?

  • A disability doesn't automatically mean a student is eligible for on-site supervision.
  • If your child has a disability and this disability makes it not possible to learn from home, then they will be considered elibible to be supervised on-site. (Please contact the school if you are unsure)

The office is open throughout the remote and flexible learning. If you have queries or concerns, please contact us.
